major clutch issues

rite so in the past month ive put 2 clutches in my saxo westcoast and it has destroyed both. The center part of the friction plate on the first just split open the second took the entire centre out. anyone got any ideas what could be causing this or seen this before. the pics below is of the latest one.

can you take apic of the flywheel and all other bits to do with it to see what state they are in?

ive found one of the little pegs on the release bearing stopping it coming off the fork has broken off. also on the inside of the pressure plate it looks like it has been pushed to far in because its marked on the inside. im thinking the release bearing came off the fork and has pushed it too far maybe? i will get some more pics tomos but theres no obvious damage or scores in the flywheel.
